During preparation activities for removal of damaged CIT nightshift supervisor instructed relocation of plastic mattress to aid removal of damaged CIT allowing the mattress to be closer to the CIT position.
During removal risk assessments and briefings, movement of the mattress was not discussed. However shift supervisor spotted an opportunity to improve removal operations but failed to consult the permit allowance tracker prior to initiating the change. Following relocation shift supervisor noted the error and raised to the OM and Client Rep. Subsequently the client rep and onshore re-iterated the importance of permit compliance and performed a check on remaining laydown allowances to ensure appropriate for forthcoming operations.”
Shell raised this onshore to ensure the importance of permit compliance in future record a lesson learnt at lessons learnt session. |
